To expedite your check-in process at the Pet Resort, please take a few minutes to print the applicable forms below, fill in all necessary information, and bring the completeted forms with you.

Boarding Agreement
A completed and signed Boarding Agreement is required for each boarding visit. Please complete all information.  All blanks must be filled in; if a particular item is not applicable, please indicate n/a.

Sunday Pick Up Policy
For our clients' convenience, Greenbelt Pet Resort offers pick up of resort guests on Sunday afternoons at 5:00 pm in accordance with this Policy.
NOTE:  Holiday Weekend Pick-Up is scheduled 5:00 pm on day before Resort reopens, or as determined by management.

Canine Pet Profile 
A pet profile is required on every guest staying at the Resort.  This form gives our staff insight as to your pet's personality, likes and dislikes.  You will only need to complete this form once; afterwards, please inform the staff of any changes in your pet's behavior.

Feline Pet Profile
A pet profile is required on every guest staying at the Resort.  This form gives our staff insight as to your pet's personality, likes and dislikes.  You will only need to complete this form once; afterwards, please inform the staff of any changes in your pet's behavior.

PlayGroup Agreement
This Agreement allows our staff to pursue PlayGroup interaction for your pet.

After reviewing your pet's Profile, our staff will spend one-one-one time with your pet to determine temperament and response to commands.  Dogs are then monitored in outside runs; this enables the dogs to interact, even though they are separated by fences.  If no signs of aggression are detected, dogs of similar size and temperament will be introduced to each other with an Associate handling each animal on a leash to prevent any unexpected actions on the part of one or both dogs.  The staff will keep notes on which animals play well with other animals.  These notes will enable the resort staff to put together a list of pets approved for PlayGroups.
